Home Delivered Meals
Hot, home-cooked, nutritionally balanced meals are available to homebound individuals in southern Ashtabula and northern Trumbull Counties. Call for eligibility requirements. Meals are prepared in the Country Neighbor kitchen Monday through Friday and delivered hot. Frozen meal packs are available for weekends and holidays. Monthly menus are available.
Hot, home-cooked, nutritionally balanced meals are service Monday through Friday in Country Neighbor’s Buckeye room. Monthly menus are available.
The personal care primarily provides basic health maintenance and/or personal hygiene assistance to individuals in their homes. Personal care service assists participants to achieve optimal function with activities of daily living and instrumental activities of daily living. Personal care services may include respite for a participant’s caregiver.
Tasks in this service include, but are not limited to:
Reading and recording the participant’s temperature, pulse and respiration.
Providing bathroom care, emptying and cleaning the bedside commode and urinary catheter bags. Changing bed linens, removing trash, and washing, drying, folding, ironing and putting away participant’s laundry.
Performing personal hygiene and grooming assistance using bed, tub, shower, and partial bath techniques. Shampooing the participant’s hair in the sink, tub, or bed. Providing nail and skin care. Assisting the participant with oral hygiene, toileting and eliminations.
The homemaker program provides services which enable participants to achieve and maintain clean, safe, and healthy environments. Tasks in this service include, but are not limited to:
Performing house cleaning including dusting furniture; sweeping, vacuuming, and washing floors; kitchen care including washing dishes, appliances, and counters; washing inside windows within reach from the floor; and removing trash.
Maintaining a clean bathroom including the tub, show, sink, toilet bowl, commode chair/urinal, mirrors, and tub and shower lining.
Planning and preparing the participant’s meals, including special diets.
Changing bed linens; and washing, drying, folding, ironing, and putting away the participant’s laundry.
Transportation to and from Country Neighbor, medical appointments, recreational activities, social service agency visits, and limited shopping for residents 60 years old and older in southern Ashtabula. Country Neighbor operates 14 vans of which 7 are equipped with wheelchair lifts. Appointments must be made with Country Neighbor staff, generally, 1 to 2 weeks prior to date of request.
Prescription Assistance and Medicare Counseling
The Prescription Assistance for Senior’s program helps individuals 60 and older who are having difficulty paying for their prescription medications. If you are under 60, call for information. Individuals who have no prescription drug coverage, or who have a Medicare Part D plan but still have high out of pocket expenses may be able to obtain their prescription medications at little or no cost. Most manufacturers of brand name medications have programs that provide their medications at no cost. A person can apply for help with a specific brand name medication by completing the manufacturer’s patient assistance application and supplying required documentation about income and any prescription drug plan they might have. Application forms and required documentation varies from company to company which makes it hard for the average individual to take advantage of these programs. That is why the Patient Assistance for Seniors Program was created in 2000. We are familiar with each company’s eligibility requirements and the forms and documentation that must be submitted. We guide applicants through an often confusing and time consuming process so they can obtain vital brand name prescription medications. Several companies will provide their medications to Medicare Part D recipients but special requirements apply and specific applications and documentation must be submitted. We also help many individuals obtain generic medications at reduced cost, often for as little as $20.00 for a six month supply. The Prescription Assistance for Senior’s program is available to residents 60 years of age and older who live in southern Ashtabula and northern Trumbull County.
Country Neighbor’s Medicare Counseling program provides information and assistance to individuals 60 years and older who will soon be eligible Medicare, or who have questions about how their Medicare coverage works. We cover issues related to Medicare Part A, B, C and D. We provide information about Medicare Advantage Plans (Medicare Part C), traditional Medigap policies, and Medicare Part D Prescription drug plans. We also help individuals submit application form for Medicare Part D Extra Help. Those who qualify for extra help will have reduced premiums, deductibles and co-pays for their prescription Medications.
Every year during the Medicare Part D open enrollment Period, which runs from Oct. 15 – Dec 7, we help Medicare Part D recipients find insurance plans for the following year that will provide the cheapest and most comprehensive prescription drug (Medicare Part D) coverage.
