Transportation

This service is available to residents 60 years old or older residing in Ashtabula or northern Trumbull Counties to medical appointments, financial institutions, grocery shopping, nutrition sites, and social activities. Funding sources have specific requirements depending upon the need.

Our current fleet has three standard minivans, three modified minivans, and four handicap accessible vans.

Transportation is available to destinations in Ashtabula, Trumbull, Lake, Geauga, Cuyahoga, and neighboring counties in Pennsylvania. Generally, vehicles operate 4:00 a.m. to 7:00 p.m., Monday through Friday and Saturdays 4:00 a.m. to 3:00 p.m. However, exceptions may be made dependent upon passenger needs.

Personal Care

This service is available for residents, 60 years old or older, residing in southern Ashtabula and northern Trumbull Counties. Personal care services include assisting the client with bathing, eating, dressing, personal hygiene, grooming, and other activities of daily living and instrumental activities of daily living.

The aide may also do housekeeping tasks related to the personal care such as cleaning the bathroom, including tub/shower, commode and floor, changing the linens on the bed, laundry, cleaning the eating and food preparations areas.

Homemaker

Homemaker services include tasks such as dusting and straightening furniture, cleaning floors or rugs by a wet or dry mop or vacuum, cleaning the kitchen by washing dishes, pots, pans, and the outside of appliances, counters and cabinets. The homemaker may also clean the tub, shower, sink, toilet, urinal or bedside commode; change bed linens, and make beds; wash inside windows or mirrors; and remove trash from the home.

He/she may do laundry including washing and drying clothes, folding and ironing clothes and linens, and putting away finished laundry.

Chore Services

This service is available to individuals over the age of 60 residing in southern Ashtabula and northern Trumbull Counties. The handyman performs lawn mowing, snow removal and minor home repairs such as changing light bulbs, fixing leaky faucets, broken boards on the steps, putting up and maintaining smoke/CO detectors, changing window screens and storm windows with seasonal weather, putting in a new mailbox, winterizing air conditioners, etc.

Jobs not included in handyman are tasks that require licensed professionals such as, electrical such as wiring, roof replacement, etc.

Home Delivered Meals

Home delivered meals are available to homebound individuals over the age of 60 residing in southern Ashtabula and northern Trumbull Counties. We will deliver one hot meal Monday through Friday. Alternate meal choices are offered based on individual needs.

All meals are planned to meet 1/3 of the RDA (Recommended Daily Allowance) of vitamins and minerals. The menus are approved and signed off by a Registered Dietician.

Grocery Shopping

This service is available to individuals age 60 or older residing in southern Ashtabula and northern Trumbull Counties. Country Neighbor’s Outreach Specialist will assist with shopping for groceries and delivering them to the client’s home.

Health and Wellness Programs

Country Neighbor Program, Inc. provides many programs and activities to help maintain health and wellness in older adults. Check out our Activities Calendar on our website.

Medicare Counseling

Country Neighbor’s Outreach Specialist is trained through the Ohio Department of Insurance’s Senior Health Insurance Information Program to assist seniors in determining Part D plans, Advantage plans, Medicare supplemental plans, Low Income Subsidy (extra help) and billing issues.

Our trained staff can also assist with other issues such as Medicaid/Food Stamp applications, assistance in completing forms and documents, etc.

Prescription Assistance

The Outreach Specialist can assist with obtaining free medications through the pharmaceutical companies at no cost to the senior. Our program has been in existence since 2000 and has been successful in obtaining millions of dollars of free medications for Ashtabula and Trumbull County older adults. Many of the drug companies will provide free medications even if you currently have Medicare Part D coverage, Medicare Advantage plan or group insurance.
